Serves children ages 18 months to 18 years. Requires referral from Primary Care Physician. Accepts Apple Health/ Medicaid and most private insurances.

Connected Care

  • Medication management and collaboration with your primary care provider
  • Support for accessing care and close collaboration with community health care providers and therapists to meet the specific needs of each child
  • Triaging services by a family support coordinator and continued monitoring of care access
  • Standard of care six-month follow-up consultation with team
